Divide it into categories like:
school
friends
family
activities
personal support
Then add:
one person
what they are helpful for
one sentence you could realistically say to them
Example:
Math teacher — explaining assignments — “Can I ask about something confusing from class?”
You do not need a huge support system. Most students just need to stop feeling like they have to figure out everything alone. This list can keep growing bit by bit.
